Background:

  • Nonpharmacologic interventions are complex and often combined with drugs in rheumatic disease treatment.
  • Treatment strategies for rheumatic diseases are complex interventions requiring evaluation of processes and outcomes.
  • Existing outcome measures in pharmacological research are insufficient for assessing nonpharmacologic contributions.

Framework:

  • The CARE network surveyed health professionals and patients (2008-2010) to identify core outcomes from the International Classification of Functioning, Disability and Health (ICF) perspective.
  • This provided new information on domains for evaluating complex interventions.

Implementation:

  • Outcome Measures in Rheumatology Clinical Trials (OMERACT) participants identified limitations in current outcome measures for assessing nonpharmacologic contributions.
  • Existing outcomes are insufficient for evaluating complex interventions in rheumatology.

Implications:

  • Further research is needed to develop and validate new outcome domains for complex interventions in rheumatic diseases.
  • Recommendations are proposed for developing research in outcome evaluation for complex interventions in rheumatology.
