Background:

Observation:

  • A 45-year-old female presented with acute confusion, memory loss, and incontinence.
  • Clinical findings included Parinaud's phenomenon and dysarthric speech.
  • Imaging revealed a large tectal mass with hydrocephalus; Magnetic Resonance Spectroscopy (MRS) indicated an aggressive neoplasm.

Findings:

  • Pathology confirmed Glioblastoma Multiforme (GBM) in the tectal region.
  • Magnetic Resonance Spectroscopy (MRS) demonstrated elevated choline/N-acetylaspartate ratio and increased lactate, indicative of malignancy.
  • The patient underwent initial hydrocephalus management followed by tumor debulking and biopsy.

Implications:

  • High-grade lesions like GBM can occur in the tectal plate, challenging traditional assumptions.
  • MRS is a valuable tool for differentiating aggressive tectal plate lesions from benign ones.
  • Biopsy is essential for accurate diagnosis, prognosis, and treatment planning of tectal plate masses.
