Summary

Binocular blindness during migraine is rare, affecting 1.6% of patients. This condition predominantly impacts women and may indicate an underlying clotting disorder, warranting further investigation.

  • Migraine patients rarely report bilateral blindness during headache attacks.
  • Limited research exists on the prevalence, demographics, and diagnostic findings of binocular blindness in migraine.

Observation:

  • A study at a headache center identified 6 patients (1.6%) with binocular blindness during migraine.
  • All affected patients were female, with a history of migraine for at least 3 years, often without aura.

Findings:

  • Episodes of binocular blindness varied in duration and frequency, occurring in isolation during headaches.
  • Neuroimaging was normal in all patients, but coagulopathy testing was abnormal in all.
  • Abnormalities included MTHFR polymorphisms and a positive lupus anticoagulant in one patient.
