Hyperthyroid vs hypothyroid eye disease: the same severity and activity

Thyroid eye disease (TED) presents similarly in patients with hyperthyroidism and hypothyroidism. Demographics, severity, and activity scores were comparable between the two groups, indicating a shared disease category.

Background:
- Thyroid eye disease (TED) is an autoimmune condition affecting the eyes, often associated with thyroid dysfunction.
- It can occur in both hyperthyroid and hypothyroid states, but its characteristics in these different contexts are not always clearly defined.
Purpose of the Study:
- To compare the demographic profiles, clinical severity, and disease activity of TED in patients with hyperthyroidism (Hr-TED) versus primary hypothyroidism (Ho-TED).
Main Methods:
- A cross-sectional study was conducted involving patients with TED referred from an endocrinology clinic between 2003 and 2006.
- Data collected included patient demographics, a comprehensive eye examination, and standardized scoring systems: NOSPECS, total eye score, clinical activity score, and Rundle grading.

- Out of 851 patients, 303 (35.6%) had TED. The majority (92.4%) had Graves' hyperthyroidism (Hr-TED), while 7.5% had primary hypothyroidism (Ho-TED).
- No significant differences were observed in age, gender, severity scores, activity scores, Rundle grade, unilateral presentation, smoking habits, duration of eye disease, or time from thyroid diagnosis to TED onset between Hr-TED and Ho-TED groups.
- However, the mean duration of thyroid disease was significantly longer in the Hr-TED group (49.6 months) compared to the Ho-TED group (22.7 months).
Conclusions:
- The similar demographics and clinical characteristics, including severity and activity scores, suggest that TED in hyperthyroid and hypothyroid patients represents the same disease category.
- This finding implies that management strategies for TED may not need to be significantly differentiated based on the underlying thyroid status.
