Cisplatin chemotherapy and surgery cure most germ cell tumors. High-dose chemotherapy with stem cell support shows promise for relapsed or chemotherapy-resistant germ cell tumors, with a new trial underway.
Area of Science:
- Oncology
- Hematology
Context:
- Germ cell tumors (GCTs) are typically curable with cisplatin-based chemotherapy and surgery.
- However, patients with initial poor response or relapsed disease face a poor prognosis.
Purpose:
- To evaluate high-dose chemotherapy with hematopoietic stem cell support (HDC-HSCT) as a therapeutic approach for GCTs.
- To inform clinical practice regarding the role of HDC-HSCT in GCT treatment algorithms.
Summary:
- HDC-HSCT has been investigated in numerous Phase II and III trials for GCTs.
- Despite existing data, HDC-HSCT is not yet a standard first-line or first-salvage treatment option.
- A new randomized Phase III trial for relapsed GCT patients has been initiated.
Impact:
- The initiation of a randomized trial aims to clarify the efficacy and role of HDC-HSCT in relapsed GCTs.
- Findings may lead to the integration of HDC-HSCT into treatment guidelines for specific GCT patient populations.
- This research addresses a critical unmet need for improved therapies in refractory and relapsed GCT cases.
