Abstract:
The entry of drugs and other biological compounds into the brain is a process that can be considered separately from the effects of those drugs and compounds once they have entered the brain. The rate of entry of compounds into the brain may occur either by passive diffusion or by carrier-mediated transport. Many processes interact to affect the amount of the compound that eventually enters the brain, including protein binding, systemic metabolism, and local metabolism and diffusion within the brain.

The blood endothelial barrier is the most porous of these. It allows all small ionized, un-ionized, and lipophilic molecules to pass through the endothelial lining into the interstitial space...

Factors Affecting Drug Distribution: Physiological Barriers
The capillary endothelial barrier allows only smaller molecules below 600 Da (Daltons) to pass through. It also restricts drugs like heparin that are bound to blood components, limiting their movement within the bloodstream.
