Summary

Hepatitis delta virus (HDV) superinfection in patients with chronic hepatitis B virus (HBV) can cause severe hepatitis. A low HBV-DNA level with high ALT may indicate HDV, treatable with peginterferon-alpha.

Background:

  • Hepatitis delta virus (HDV) is an RNA virus requiring hepatitis B virus (HBV) for infection.
  • HDV superinfection affects 5% of the 350 million globally HBV-infected individuals, leading to higher morbidity and mortality.
  • Distinguishing HDV from HBV infection is clinically challenging, relying on antibody and HDV-RNA detection via PCR.

Observation:

  • A 37-year-old male with chronic HBV presented with elevated ALT levels, suggesting severe hepatitis.
  • Low HBV-DNA levels indicated that HBV alone could not account for the observed inflammation.
  • Further diagnostic tests confirmed an HDV superinfection.

Findings:

  • Severe hepatitis, characterized by high ALT levels, coupled with low HBV-DNA particle levels, is a potential indicator of HDV superinfection.
  • The patient with HDV superinfection was successfully treated with peginterferon-alpha.

Implications:

  • This case highlights the importance of considering HDV superinfection in HBV patients with severe hepatitis and low viral load.
  • Early diagnosis and treatment with peginterferon-alpha can effectively manage HDV superinfection.
