Summary

Astigmatism changes induced by contact lenses slightly affected retinal nerve fiber layer (RNFL) thickness measurements using Cirrus HD OCT, but not macular thickness. This suggests considering astigmatism when interpreting RNFL thickness in OCT scans.

Background:

  • Accurate measurement of retinal nerve fiber layer (RNFL) and macular thickness is crucial for diagnosing and monitoring ocular diseases.
  • Astigmatism, a common refractive error, may influence the precision of optical coherence tomography (OCT) measurements.

Purpose of the Study:

  • To investigate the impact of induced astigmatism on RNFL and macular thickness measurements obtained via Cirrus HD spectral-domain optical coherence tomography (OCT).

Main Methods:

  • Healthy subjects' eyes underwent RNFL and macular thickness assessments using Cirrus HD OCT at baseline.
  • With-the-rule (WTR) and against-the-rule (ATR) astigmatism were induced using toric soft contact lenses.
  • Differences in measurements between baseline and induced astigmatism states were analyzed.

Main Results:

  • Induced WTR and ATR astigmatism (mean 2.92 and 3.18 diopters) led to statistically significant decreases in various RNFL thickness measurements (P < 0.05).
  • Macular thickness measurements remained unaffected by the induced astigmatism (P ≥ 0.250).

Conclusions:

  • Astigmatism changes, even when induced by contact lenses, can subtly alter RNFL thickness readings from Cirrus HD OCT.
  • Clinicians should consider the potential influence of astigmatism on RNFL measurements, particularly in individuals with higher degrees of astigmatism.
