The superior transseptal approach for mitral valve surgery offers excellent exposure. A new technique preserves the sinus node artery, reducing postoperative atrial dysrhythmia and sinus node dysfunction.

Background:

The superior transseptal approach is a standard technique for mitral valve surgery, providing optimal visualization and minimizing mitral valve deformation.
Despite its advantages, postoperative atrial dysrhythmia and sinus node dysfunction remain occasional complications.

Observation:

Routine mitral valvoplasty via the superior transseptal approach can lead to unexpected atrial dysrhythmias.
The proximity of the sinus node artery to the surgical site poses a risk during left atrial incision.

Findings:

A simple surgical technique was developed to preserve the sinus node artery during the superior transseptal approach.
This technique involves careful dissection and preservation of the sinus node artery during left atrial dome incision.
