Summary

Cecal volvulus in pregnancy is rare and hard to diagnose, often leading to delayed treatment. Early suspicion and laparoscopic surgery can ensure safe and successful outcomes for mother and child.

Background:

Intestinal obstruction during pregnancy is uncommon, with colonic volvulus accounting for 24% of cases.
Delayed diagnosis is frequent due to the rarity of the condition and limitations in imaging modalities during pregnancy.

Observation:

A 33-year-old pregnant patient with twins presented with acute abdominal pain.
Post-cesarean delivery, she developed severe right lower quadrant pain, nausea, vomiting, and abdominal distension, suggestive of bowel obstruction.
Imaging revealed a possible cecal volvulus, leading to surgical intervention.

Findings:

Laparoscopic detorsion of the cecal volvulus and appendectomy were performed.
The patient experienced an uneventful postoperative recovery and was discharged on the fourth day.

Implications:

Cecal volvulus in pregnancy presents diagnostic challenges and carries risks of high maternal and fetal morbidity/mortality if diagnosis is delayed.
A high clinical suspicion is crucial for pregnant or postpartum women with signs of bowel obstruction.
Laparoscopy offers a safe and effective surgical approach when diagnosed promptly.
