    Summary

    Peripheral odontogenic fibroma (POF) is a rare benign gingival lesion. This case report details a recurrent POF in a healthy male, successfully treated with surgical excision and a free palatal graft for optimal esthetics.

    Background:

    • Peripheral odontogenic fibroma (POF) is an uncommon extraosseous variant of central odontogenic fibroma.

    • Clinically, POF can be misdiagnosed, often confused with peripheral ossifying fibroma.

    Observation:

    • A case of recurrent peripheral odontogenic fibroma (POF) in the right maxilla of a healthy 32-year-old male is presented.
    • The lesion mimicked other benign neoplasms and reactive lesions.
    • The patient had a history of recurrent POF.

    Findings:

    • Surgical excision was performed for the recurrent POF.
    • The surgical defect was reconstructed using a free palatal graft.
    • The treatment resulted in a favorable esthetic outcome.

    Implications:

    • This case highlights the importance of accurate diagnosis of peripheral odontogenic fibroma.
    • Free palatal grafts can provide favorable esthetic results in the management of gingival lesions.
    • Understanding the clinical presentation and treatment options for recurrent POF is crucial for oral surgeons and pathologists.
