Background:

Ciliated hepatic foregut cysts (CHFCs) are uncommon congenital cystic lesions of the liver.
Typically solitary, unilocular, and subcapsular, CHFCs often present asymptomatically.
Their mucoid contents can complicate imaging interpretation and diagnosis.

Observation:

A 69-year-old female presented with obstructive jaundice.
A 25 mm cystic lesion in the porta hepatis adhered to hepatic ducts, causing extrinsic compression.
The lesion's location and presentation mimicked a biliary cystic neoplasm.

Findings:

Surgical resection via extended left hepatectomy was performed.
Histological examination confirmed the diagnosis of CHFC.
The cyst's adherence and compression of the common hepatic duct were noted.

Implications:

CHFCs, though rare, must be considered in the differential diagnosis of liver cystic lesions.
This case underscores the potential for CHFCs to cause significant biliary obstruction.
Accurate preoperative diagnosis remains challenging, necessitating a high index of suspicion.
