Area of Science:

  • Public Health
  • Infectious Disease Epidemiology
  • Behavioral Science

Background:

  • Injection drug users (IDUs) in China engage in high-risk sexual behaviors, facilitating HIV transmission.
  • Drug rehabilitation centers represent an overlooked opportunity for crucial sexual health education for IDUs.

Purpose of the Study:

  • To evaluate the effectiveness of a peer-based HIV/STI prevention intervention for injection drug users in South China's rehabilitation centers.

Main Methods:

  • A convenience sample of 667 drug users was recruited from two rehabilitation centers.
  • An intervention group (n=247) received peer-based education, while a control group (n=420) received routine education.
  • HIV/STI knowledge and behaviors were assessed at 3 months and 2-23 months post-intervention.

Main Results:

  • Intervention group participants reported significantly higher condom use with casual partners (60.0% vs. 12.5%) at the second follow-up.
  • Participants in the intervention group also reported less frequent daily injection (56.7% vs. 26.4%) at the second follow-up.
  • Substantial loss to follow-up occurred, associated with distance from the center and baseline HIV knowledge.

Conclusions:

  • Rehabilitation centers can serve as effective sites for behavioral HIV/STI prevention services.
  • Referral to community-based programs upon release is crucial for sustained behavioral change.
  • Further research is needed to address the complex needs of detained drug users in China.
