Summary

Malignant paraganglioma can spread to the choroid, presenting as a nonpigmented mass. Stereotactic radiotherapy effectively treated this rare ocular metastasis in a patient with widespread disease.

Area of Science:

  • Ophthalmology
  • Oncology
  • Radiology

Background:

  • Malignant paraganglioma is a rare tumor with potential for metastasis.
  • Choroidal metastases are uncommon, and paraganglioma is an even rarer cause.
  • This case highlights the importance of considering rare diagnoses in ophthalmology.

Observation:

  • A 50-year-old male presented with a nonpigmented choroidal mass and retinal detachment.
  • Biopsy confirmed paraganglioma, with metastatic disease found in the vertebrae, mediastinum, and lungs.
  • The primary tumor origin remained unidentified.

Findings:

  • Stereotactic radiotherapy led to regression of the ocular tumor.
  • Recurrent disease in the contralateral eye also responded to radiotherapy.

  • This represents the first reported case of choroidal paraganglioma.
  • Implications:

    • Malignant paraganglioma should be included in the differential diagnosis for nonpigmented choroidal masses.
    • Stereotactic radiation therapy demonstrates efficacy in managing choroidal paraganglioma.
    • This case expands the understanding of rare metastatic patterns in paraganglioma.
