Summary

A massive 46 Kg dedifferentiated liposarcoma, growing over 25 years, was surgically removed from a 72-year-old patient. This rare abdominal tumor required complex surgical intervention and ureteral stenting.

Area of Science:

  • Oncology
  • Surgical Pathology

Background:

  • Retroperitoneal tumors are rare and can grow to massive sizes over decades.
  • Dedifferentiated liposarcoma is an aggressive soft tissue sarcoma.
  • Large abdominal masses pose significant surgical challenges, including potential organ compression.

Observation:

  • A 72-year-old female presented with a 25-year history of a gradually enlarging abdominal mass, rapidly increasing over the last 5 years.
  • Imaging revealed a huge retroperitoneal tumor encasing tortuous ureters and small lung nodules.
  • A 46 Kg tumor was surgically excised piecemeal after ureteral stenting.

Findings:

  • Histopathology confirmed the excised mass as dedifferentiated liposarcoma.
  • The patient experienced a prolonged ileus for 5 days post-surgery.

  • Short-term outcomes were positive, with the patient ambulating with support.
  • Implications:

    • This case highlights the possibility of extreme tumor growth in the retroperitoneum.
    • Surgical management of massive liposarcomas requires careful planning, including ureteral stenting.
    • Despite the tumor's size and aggressive nature, successful surgical debulking can lead to improved patient function.
