Surgical Techniques

Background:

The retrosigmoid approach is a common surgical technique for addressing posterior fossa pathologies.
Multiple patient positioning variations exist for this approach, each with potential benefits and drawbacks.

Purpose of the Study:

To introduce and evaluate a simple, safe, and quick patient positioning technique for the retrosigmoid approach.
To optimize patient safety, surgeon comfort, and intraoperative visualization during posterior fossa surgery.

Main Methods:

A retrospective review of a prospective surgical database was conducted.
Data from over 970 retrosigmoid operations performed over 19 years were analyzed for complications and exposure difficulties.

Main Results:

No positioning-related complications were reported in any of the 970+ procedures.
Surgeries were never aborted due to inadequate surgical exposure.
No instances of normal cerebellum resection or posterior fossa retractor use were recorded.

Conclusions:

Supine positioning represents a safe and effective alternative positioning strategy for the retrosigmoid approach.
This technique enhances surgical outcomes and patient safety in posterior fossa operations.
