Anatomy of the Eyeball01:20

Anatomy of the Eyeball

The eye is a spherical, hollow structure composed of three tissue layers. The outer layer — the fibrous tunic, comprises the sclera — a white structure — and the cornea, which is transparent. The sclera encompasses some of the ocular surface, most of which is not visible. However, the 'white of the eye' is distinctively visible in humans compared to other species. The cornea, a clear covering at the front of the eye, enables light penetration. The eye's middle layer, the vascular tunic,...

Summary

Spectral-domain optical coherence tomography (SD-OCT) shows good correlation with time-domain (TD)-OCT for retinal nerve fiber layer (RNFL) thickness measurements. A generalized estimating equation mixed model offers superior correlation compared to Pearson analysis for glaucoma patients.

Area of Science:

  • Ophthalmology
  • Medical Imaging
  • Glaucoma Research

Background:

  • Spectral-domain optical coherence tomography (SD-OCT) is poised to supersede time-domain optical coherence tomography (TD-OCT).
  • Accurate retinal nerve fiber layer (RNFL) thickness measurement is crucial for glaucoma diagnosis and monitoring.
  • Evaluating the concordance between SD-OCT and TD-OCT is essential for clinical practice transitions.

Purpose of the Study:

  • To assess the correlation between RNFL thickness measurements from SD-OCT and TD-OCT devices.
  • To compare the statistical methods for evaluating this correlation in a Chinese population.
  • To inform clinical decisions regarding longitudinal RNFL monitoring across different OCT technologies.

Main Methods:

  • Prospective, cross-sectional study involving 179 participants (glaucoma, suspected glaucoma, ocular hypertension, normal subjects).
  • Simultaneous imaging using Stratus TD-OCT and Cirrus SD-OCT by a single operator.
  • Statistical analysis included Pearson correlation and generalized estimating equation (GEE) mixed effect models, adjusting for individual variation.

Main Results:

  • A strong correlation was observed between RNFL thickness measurements from both OCT devices.
  • The GEE mixed model demonstrated a superior correlation (0.9470) compared to Pearson analysis (0.8798) for average RNFL thickness.
  • Good correlation was found across all RNFL thickness parameters evaluated.

Conclusions:

  • RNFL thickness measurements from TD-OCT and SD-OCT systems exhibit good correlation.
  • The GEE mixed model provides a more robust correlation assessment than Pearson analysis.
  • Clinicians must exercise caution when interpreting longitudinal RNFL data from different OCT systems.
