[Experience with open reduction of congenital hip dislocation by the Ludloff method]
Insights
Simple open reduction for congenital hip dislocation in children under 3 years showed favorable acetabular development in younger patients. This Ludloff procedure is a key treatment stage for older children.
Area of Science:
- Orthopedic Surgery
- Pediatric Orthopedics
Background:
- Congenital dislocation of the hip (CDH) is a common pediatric orthopedic condition.
- Early intervention is crucial for optimal outcomes in CDH treatment.
Purpose of the Study:
- To evaluate the efficacy of simple open reduction using the Ludloff method for treating congenital dislocation of the hip in children.
- To assess the impact of the procedure on acetabular development based on patient age and initial acetabular index.
Main Methods:
- The study involved 54 children aged 3 months to 3 years undergoing simple open reduction of congenital hip dislocation via the Ludloff approach.
- A total of 79 hip joints were operated on.
- Patients were categorized by age (under 1-2 years vs. 2-3 years) and initial acetabular index (below 30-40 degrees).
Main Results:
- Favorable acetabular development was observed in children operated on before 1-2 years of age with an initial acetabular index below 30-40 degrees.
- In children aged 2-3 years, the procedure was considered a stage within a broader treatment plan for CDH.
Conclusions:
- Simple open reduction following the Ludloff technique is effective for improving acetabular development in younger children with CDH.
- For older children (2-3 years), this method serves as an important component in the multi-stage management of congenital hip dislocation.
Abstract:
The authors have performed simple open reduction of congenital dislocation of the hip after Ludloff in 54 children aged 3 months to 3 years. The total number of the operated joints was 79. The authors believe that this method provides favourable development of the acetabulum in the children operated before 1-2 years of age with the initial acetabular index below 30-40 degrees. In the children aged 2-3 years it should be regarded as one of the stages in the treatment of congenital dislocation of the hip.
