Background:

  • Arthrobacter-like organisms, including Pseudoclavibacter, are infrequently implicated in human infections.
  • Human infections caused by Pseudoclavibacter are poorly documented.

Purpose of the Study:

  • To report a case of human subcutaneous infection caused by a Pseudoclavibacter-like organism.
  • To highlight the diagnostic methods for identifying such rare pathogens.

Main Methods:

  • A case study of an 81-year-old man with a chronic wound infection was analyzed.
  • Skin biopsy and subcutaneous swab cultures were performed.
  • 16S rRNA gene sequencing and matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) were used for bacterial identification.

Main Results:

  • The patient developed a subcutaneous infection despite treatment with clindamycin and metronidazole.
  • Microscopic examination of the subcutaneous swab revealed Gram-positive rods.
  • Genetic analysis identified the Gram-positive rod as a Pseudoclavibacter-like organism with 99% sequence similarity to uncultured strains.
  • MALDI-TOF MS confirmed the uniqueness of the isolate.

Conclusions:

  • Pseudoclavibacter-like organisms can be identified in human cutaneous and subcutaneous infections.
  • This case expands the known spectrum of human pathogens to include Pseudoclavibacter species.
