Summary

Prophylactic intrapartum amnioinfusion significantly reduced fetal distress during labor for women with oligohydramnios. This intervention improved fetal outcomes and decreased the need for operative delivery.

Area of Science:

  • Obstetrics and Gynecology
  • Fetal Medicine
  • Intrapartum Care

Background:

  • Oligohydramnios, defined as an amniotic fluid index (AFI) ≤ 5.0 cm, is associated with increased intrapartum fetal risks.
  • Previous interventions for oligohydramnios have shown limited success in improving fetal outcomes during labor.

Purpose of the Study:

  • To evaluate the efficacy of prophylactic intrapartum amnioinfusion in reducing intrapartum fetal morbidity in patients with oligohydramnios.
  • To assess the impact of amnioinfusion on fetal heart rate patterns, meconium passage, and operative delivery rates.

Main Methods:

  • A prospective, randomized trial involving 60 women in the latent phase of labor with oligohydramnios (AFI ≤ 5.0 cm) and otherwise normal fetal status.
  • Patients were randomized into an amnioinfusion group (n=30) or a control group (n=30).

Related Experiment Videos

  • The amnioinfusion group received continuous infusion to maintain AFI ≥ 8.0 cm throughout labor.
  • Main Results:

    • The amnioinfusion group demonstrated significantly lower rates of meconium passage (p=0.04) and severe variable decelerations (p=0.04).
    • End-stage bradycardia (p=0.05) and operative delivery for fetal distress (p=0.002) were also significantly reduced in the amnioinfusion group.
    • Higher umbilical arterial blood pH values (p=0.02) were observed in neonates whose mothers received amnioinfusion.

    Conclusions:

    • Prophylactic intrapartum amnioinfusion is an effective technique for mitigating intrapartum fetal distress in the setting of oligohydramnios.
    • This intervention can lead to improved fetal well-being and a reduced need for emergency operative interventions.
    • Amnioinfusion represents a valuable tool for enhancing fetal safety during labor and delivery.
