Lymphatic staining significantly increases the number of lymph nodes (LNs) detected in colorectal cancer specimens. This method improves lymph node harvest and reduces understaging, crucial for accurate cancer diagnosis.

Area of Science:

  • Oncology
  • Surgical Pathology
  • Cancer Staging

Background:

  • Accurate lymph node (LN) assessment is critical for colorectal cancer staging and treatment.
  • Suboptimal LN harvest can lead to understaging and affect patient prognosis.

Purpose of the Study:

  • To evaluate the impact of lymphatic staining on lymph node yield in colorectal cancer specimens.
  • To determine if lymphatic staining improves the accuracy of lymph node staging.

Main Methods:

  • A randomized study comparing lymphatic staining versus unstained methods for colorectal cancer specimens.
  • Quantification of lymph node retrieval, size, and detection of metastases.

Main Results:

  • Lymphatic staining significantly increased the mean LN harvest (30±12 vs. 13±5 nodes).

  • Insufficient LN harvest (<12 nodes) was drastically reduced with staining (1 vs. 14 cases).
  • More small LNs (<2mm and 2-5mm) were detected using the staining method.
  • Conclusions:

    • Lymphatic staining is an effective method to enhance lymph node detection in colorectal cancer surgery.
    • Improved LN harvest through staining leads to more accurate staging and potentially better patient outcomes.
