Management of free-floating thrombus (FFT) in the common carotid artery is challenging. Surgical intervention is recommended for symptomatic FFT, as demonstrated in this case report.
Area of Science:
- Vascular Surgery
- Neurology
- Interventional Cardiology
Background:
- Free-floating thrombus (FFT) management remains challenging, with ongoing debate regarding optimal surgical versus medical treatment strategies.
- The common carotid artery is an uncommon site for FFT, presenting unique diagnostic and therapeutic considerations.
Observation:
- An 80-year-old female presented with acute right-hand weakness and syncope, indicative of cerebrovascular compromise.
- Cervical ultrasound revealed a significant free-floating thrombus within the left common carotid artery.
Findings:
- The patient underwent immediate carotid endarterectomy with Dacron patch angioplasty for the symptomatic left common carotid artery FFT.
- The surgical procedure was completed without any immediate complications.
Implications:
- This case underscores the importance of considering surgical intervention for symptomatic FFT, particularly when located in the common carotid artery.
- Prompt interventional management of FFT in the presence of neurological symptoms can lead to favorable outcomes and prevent further embolic events.
