Background:

  • Genitourinary (GU) injuries are a potential consequence of bicycling trauma.
  • The incidence and characteristics of bicycling-related GU injuries are not well-defined.

Purpose of the Study:

  • To evaluate the incidence and patterns of genitourinary (GU) injuries in patients involved in bicycle trauma.
  • To analyze patient demographics, injury types, management, and outcomes associated with bicycling-related GU injuries.

Main Methods:

  • Data were extracted from the National Trauma Data Bank (NTDB) (version 9.1).
  • Cases were identified using Ecodes for pedal cyclists and ICD-9 codes for GU injuries.
  • Analysis included injury types, demographics, Injury Severity Score, surgical interventions, and patient disposition.

Main Results:

  • Out of 635,013 trauma cases, 16,585 involved bicycle injuries.
  • Genitourinary (GU) injuries occurred in 358 (2%) of bicycle trauma patients, predominantly males (86%) with a mean age of 29.
  • Kidneys were the most frequently injured GU organ (75%), followed by bladder/urethra (15%) and penis/scrotum (10%).
  • Associated injuries included vertebral fractures (35%) and pelvic fractures (25%).
  • Vertebral fractures were often accompanied by bladder/urethra (37.7%) or renal injuries (22.6%).

Conclusions:

  • Genitourinary (GU) injury is an uncommon complication of bicycle trauma, seen in 2% of cases.
  • Kidney injuries are the most prevalent GU injuries in bicyclists.
  • Healthcare providers should suspect concomitant renal or bladder injuries in bicyclists with vertebral fractures.
  • Young males are the primary demographic at risk for GU injuries from bicycling.
