Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. A single brachial plexus block provided long-lasting relief from phantom limb pain and cramping. This intervention also reversed cortical reorganization, offering sustained benefits for phantom limb sensations.

Background:

  • A patient with chronic, intractable phantom hand pain and cramping experienced immediate relief following a brachial plexus block.
  • The patient reported sustained pain reduction and phantom limb relaxation for six months post-procedure.

Findings:

  • Brachial plexus blockade effectively reduced phantom limb pain and cramping.
  • The intervention led to a reversal of cortical reorganization in the somatosensory cortex.
  • Long-lasting relief was observed, persisting for at least six months after a single blockade.

Implications:

  • Temporary peripheral nerve blockade may be a viable therapeutic option for managing phantom limb pain.
  • This suggests that peripheral mechanisms play a significant role in maintaining PLP and associated cortical changes.
  • Further research into regional anesthesia techniques for PLP is warranted.
