Background:

  • New daily persistent headache (NDPH) diagnostic criteria often exclude patients with prominent migrainous features.
  • This challenges the current International Classification of Headache Disorders (ICHD-2) for persistent headache diagnosis.

Purpose of the Study:

  • To investigate the characteristics and outcomes of NDPH patients with and without prominent migrainous features.
  • To compare the severity and prognosis of NDPH subtypes based on ICHD-2 criteria adherence.

Main Methods:

  • Retrospective review of 92 NDPH patients, categorized as NDPH-S (meeting ICHD-2) and NDPH-M (prominent migrainous features).
  • Data collected included demographics, headache profiles, Beck Depression Inventory (BDI), SF-36, and Migraine Disability Assessment (MIDAS).
  • Follow-up assessed outcomes over an average of 2 years.

Main Results:

  • NDPH-M comprised 64.1% of patients and showed higher headache intensity, BDI, and MIDAS scores, with lower SF-36 scores compared to NDPH-S.
  • 66% of patients achieved good outcomes (≥50% frequency reduction) after 2 years.
  • Disease duration ≤6 months and NDPH-S diagnosis predicted favorable outcomes.

Conclusions:

  • Migrainous features are prevalent in NDPH, suggesting a need to broaden diagnostic criteria.
  • NDPH-M represents a more severe subgroup with a poorer prognosis than NDPH-S.
  • Early intervention in persistent headaches may improve long-term outcomes.
