Intracytoplasmic sperm injection (ICSI) with sperm from ejaculate, percutaneous epididymal sperm aspiration (PESA), or testicular sperm extraction (TESE) provides satisfactory results for infertile couples, including azoospermic males.
Area of Science:
- Reproductive Endocrinology
- Assisted Reproductive Technology (ART)
Context:
- Intracytoplasmic sperm injection (ICSI) is a key ART for infertility.
- Azoospermic males often require sperm retrieval techniques like PESA and TESE for ICSI.
Purpose:
- To evaluate the efficacy of ICSI using ejaculated sperm versus sperm retrieved via PESA or TESE.
- To compare pregnancy and implantation rates across these different sperm sources.
Summary:
- A study of 1,436 couples undergoing ICSI found no significant differences in pregnancy (32.8% ejaculate, 43.2% PESA, 40.9% TESE) or implantation rates (11.2% ejaculate, 26% PESA, 11% TESE) between sperm sources.
- Male age showed a significant difference between PESA and ejaculated sperm groups (p=0.024).
Impact:
- ICSI combined with PESA or TESE is a viable and effective treatment for infertile couples, including those with azoospermia.
- These findings support the use of sperm retrieval techniques for ICSI in male factor infertility.
