  • The patient exhibited constant nonpulsatile tinnitus and rotatory vertigo as the primary symptoms.

Findings:

  • The audiovestibular symptoms were the sole presenting neurologic signs.
  • Rotatory nystagmus was observed, suggesting a potential central nervous system origin.

Implications:

  • Rotatory nystagmus should be considered a central sign of neurological dysfunction until definitively proven otherwise.
  • Subtle or isolated audiovestibular symptoms may indicate serious underlying brainstem pathology.
  • This case broadens the differential diagnosis for audiovestibular disturbances, emphasizing the need for thorough neurological evaluation.
