Background:

  • Previous studies indicated unrecognized HIV seroprevalence in emergency departments (EDs) ranging from 0.38% to 4%.
  • Hepatitis B (HB) poses a significant risk to emergency care providers.

Purpose of the Study:

  • To determine the seroprevalence of Human Immunodeficiency Virus (HIV) and Hepatitis B (HB) in emergency department (ED) patients.
  • To assess the feasibility of multicenter studies in emergency medicine.

Main Methods:

  • A prospective, anonymous study of HIV and HB seroprevalence was conducted on excess serum from ED patients across seven hospitals in the Portland metropolitan area.
  • Data were collected over two 48-hour periods in May and August 1988.

  • Two out of 444 patients (0.45%) tested positive for HIV, with one case being previously undiagnosed.
  • Hepatitis B core antibody (HBcAB) and Hepatitis B surface antigen (HBsAG) positivity were found in 12% and 0.6% of patients, respectively.
  • HBcAB seroprevalence correlated with race, IV drug use, and hospital location, but not with other assessed demographic or clinical factors.

Conclusions:

  • The findings support the implementation of universal body fluid precautions in emergency departments.
  • Strong advocacy for Hepatitis B vaccination for all emergency department healthcare workers is recommended.
  • Multicenter studies in emergency medicine are both desirable and feasible.
