Background:

Practice guidelines for nonmuscle invasive bladder cancer are unclear, leading to varied treatment approaches.
This variation in care does not appear to benefit patients with early-stage bladder cancer.

Purpose of the Study:

To analyze urologist practice styles for bladder cancer care.
To identify optimal patterns for using common bladder cancer services based on empirical data.

Main Methods:

Retrospective cohort study using Surveillance, Epidemiology, and End Results-Medicare linked data (1992-2005).
Analysis of elderly patients diagnosed with early-stage bladder cancer.
Multilevel models and Cox proportional hazards regression to assess the association between urologist service use and patient outcomes.

Main Results:

No clear patient benefit was observed with high-frequency cystoscopy or intravesical therapy use, regardless of disease severity.
Maximal use of urine cytology was associated with a reduced risk of bladder cancer death in patients with high-grade Ta/Tis and T1 disease.

Conclusions:

The findings suggest a need for a more individualized treatment strategy for early-stage bladder cancer.
This study demonstrates the utility of observational data in refining clinical practices when experimental evidence is limited.
