The hypothenar region's vascular supply is crucial for reconstructive surgery.
Understanding specific arterial ramifications aids in flap design.

Observation:

Anatomical dissection of 12 cadavers (24 hands) detailed the hypothenar vascular network.
Angiography in 20 hands mapped the proper palmar digital artery of the little finger.

Findings:

The proper palmar digital artery of the little finger can serve as a reverse vascular pedicle.
This allows the hypothenar skin to be raised as a viable island flap.

Implications:

This anatomical finding offers a new reconstructive option for hypothenar defects.
Successful application in three clinical cases validates the technique's potential.
