Saddle pulmonary embolism, a large clot in the main pulmonary artery, was visualized using 3D echocardiography. Successful treatment with anticoagulation led to complete clot dissolution.
Area of Science:
- Cardiology
- Radiology
- Vascular Medicine
Background:
- Saddle pulmonary embolism involves a thromboembolus at the main pulmonary artery bifurcation.
- Transthoracic echocardiography rarely visualizes pulmonary artery thrombi.
Observation:
- A case of saddle embolus in the main pulmonary artery is presented.
- Real-time three-dimensional echocardiography was used for visualization.
Findings:
- The saddle embolus was successfully visualized.
- Treatment involved intravenous unfractionated heparin followed by oral anticoagulation.
- Complete dissolution of the thrombus was achieved.
Implications:
- Real-time 3D echocardiography is a valuable tool for diagnosing saddle pulmonary embolism.
- Anticoagulation therapy can effectively lead to complete resolution of saddle pulmonary emboli.
- This case highlights the diagnostic and therapeutic potential in managing main pulmonary artery thromboemboli.

Transthoracic Echocardiography (TTE)
TTE is the most common type of echocardiogram which involves placing a transducer on the patient's chest, emitting sound waves to create heart images. TTE is invaluable for evaluating the heart's size, structure, and motion, making it particularly useful for diagnosing...
