Antiviral Nucleoside Inhibitors01:22

Antiviral Nucleoside Inhibitors

Antiviral Nucleoside InhibitorsAntiviral nucleoside inhibitors are structural analogs of natural nucleosides that interfere with viral DNA or RNA synthesis. These compounds selectively target viral polymerases due to their resemblance to host nucleosides, thereby disrupting viral genome replication.Mechanism of Acyclovir ActionAcyclovir is a guanosine analog with a three-carbon acyclic side chain. It selectively targets herpes simplex virus type 1 (HSV-1), herpes simplex virus type 2 (HSV-2),...
Genital Herpes01:23

Genital Herpes

Genital herpes is a sexually transmitted infection primarily caused by herpes simplex virus type 2 (HSV-2), though herpes simplex virus type 1 (HSV-1) is increasingly implicated in genital infections, particularly among younger populations. Transmission occurs mainly through sexual contact, with asymptomatic viral shedding serving as a major route of spread. This characteristic makes HSV-2 difficult to control at a population level, as individuals may unknowingly transmit the virus even in the...
Herpes01:28

Herpes

Herpes simplex type 1 (HSV‑1) is a widespread pathogen responsible for orolabial lesions. It is an enveloped, double-stranded DNA (dsDNA) virus belonging to the family Herpesviridae. Once the virus infects a host cell, its double‑stranded DNA genome is delivered into the nucleus, where a coordinated cascade of immediate‑early, early, and late gene expression directs viral DNA replication, structural protein synthesis, and virion assembly. After primary infection of epithelial cells, HSV-1...

Drug Dosing: Infants and Children

Pediatric patient dosages diverge from adults due to disparities in body surface area, total body water, and extracellular fluid per kilogram of body weight. The dosing regimen considers the variations in pharmacokinetics and pharmacology across distinct age groups, encompassing preterm newborns, infants, young children, older children, and adolescents. Calculation of pediatric patient doses is predicated on determining body surface area, which exhibits a superior correlation with the child's...
Cytomegalovirus Disease01:27

Cytomegalovirus Disease

Cytomegalovirus (CMV) disease is caused by human cytomegalovirus, a double-stranded DNA virus of the Herpesviridae family. While primary CMV infection is often asymptomatic in immunocompetent individuals, the virus can cause severe disease in neonates and immunocompromised patients. CMV is the most common cause of congenital viral infection in the United States, and a major pathogen in solid organ and hematopoietic stem cell transplant recipients.CMV is transmitted via bodily fluids, sexual...

Acute Pyelonephritis II: Diagnostic Studies and Management

Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...

Summary

Empiric acyclovir (ACV) is frequently used for suspected neonatal herpes simplex virus (NHSV) infection due to diagnostic challenges. While ACV appears safe and effective for NHSV, investigations are often incomplete.

Area of Science:

  • Neonatal infectious diseases
  • Pediatric neurology
  • Pharmacology

Background:

  • Neonatal herpes simplex virus (NHSV) infection diagnosis is challenging, leading to increased empiric acyclovir (ACV) use.
  • Early diagnosis and treatment are crucial for improving outcomes in neonates with suspected HSV.

Purpose of the Study:

  • To review the utilization of ACV in neonatal populations.
  • To optimize the future management strategies for NHSV infections.

Main Methods:

  • Retrospective chart review of infants treated with intravenous ACV up to 43 days of life.
  • Data collected from January 2001 to February 2007 across five hospitals in Edmonton and Calgary.
  • Analysis of indications for ACV, diagnostic investigations performed, and adverse events.

Main Results:

  • ACV was initiated for 118 infants, predominantly for possible (N=115) versus proven (N=3) HSV infection.
  • Common indications included seizures (34%), hemodynamic instability (29%), and skin lesions (24%).
  • Diagnostic workup was often incomplete, with 4% having no specimens submitted for HSV detection; ACV-related nephrotoxicity was rare (3% in unproven cases).

Conclusions:

  • Clinicians consider NHSV in neonates with seizures, instability, or skin lesions but may need to increase suspicion for earlier treatment.
  • Diagnostic investigations for NHSV are frequently incomplete.
  • Empiric ACV for suspected NHSV appears to have a favorable safety profile with uncommon adverse events.
