Background:

  • Narrow-band imaging (NBI) is effective for differentiating neoplastic from non-neoplastic colorectal polyps.
  • Accurate polyp classification is crucial for patient management and treatment decisions.

Purpose of the Study:

  • To develop and evaluate a computer-based algorithm for classifying colorectal polyps.
  • To compare the diagnostic performance of the algorithm against human experts and non-experts.

Main Methods:

  • A prospective study involving 214 patients with colorectal polyps.
  • 434 polyps (≤10 mm) were imaged using zoom NBI colonoscopy and analyzed.
  • Diagnostic performance was assessed for a computer algorithm, 2 experts, and 2 non-experts.

Main Results:

  • The computer algorithm achieved high diagnostic performance (95.0% sensitivity, 90.3% specificity, 93.1% accuracy), comparable to expert performance (93.4% sensitivity, 91.8% specificity, 92.7% accuracy).
  • Both the algorithm and experts significantly outperformed the non-expert group.
  • Analysis of smaller polyps (≤5 mm) showed comparable results.

Conclusions:

  • Computer-based classification of colon polyps can achieve high diagnostic performance.
  • While effective, a fully automatic classification system was not developed in this study.
