Skin nodules with a linear distribution - a case study.

A man developed painful, purplish nodules on his arm after an injury. Diagnosis revealed a rare infection caused by cigar-shaped organisms, identified via PAS staining.

Background:
- A 35-year-old male presented with a several-month history of developing purplish nodules on his left arm.
- The nodules appeared in a linear distribution following a minor injury and were moderately painful.
Observation:
- Dermatological examination revealed six purple nodules of varying sizes along the left arm and hand.
- Some lesions exhibited superficial ulceration and serous crusting. Enlarged axillary lymph nodes were noted.
- Systemic examination and routine laboratory tests were unremarkable. Initial cultures were negative.
Findings:
- Histopathologic examination demonstrated a mixed granulomatous and pyogenic inflammatory process.
- Periodic acid-Schiff (PAS) staining identified characteristic cigar-shaped organisms within the lesions.
Implications:
- The findings suggest a rare infectious etiology for the observed nodular lesions.
- Identification of the causative organisms is crucial for appropriate antimicrobial therapy and patient management.
