Abstract:
This case report describes a rare presentation of Streptococcus mitis endocarditis of the aortic and mitral valves, complicated by spontaneous splenic rupture due to splenic infarction, which led to massive intra-abdominal bleeding and ultimately death.
Insights
This case report details a rare instance of Streptococcus mitis endocarditis affecting heart valves, leading to spleen rupture and fatal abdominal bleeding. This highlights a severe, uncommon complication of infective endocarditis.
Area of Science:
- Cardiology
- Infectious Diseases
- Gastroenterology
Background:
- Infective endocarditis is an infection of the heart's inner lining or valves.
- Streptococcus mitis is a common bacterium, typically found in the oral cavity.
- Splenic complications from endocarditis are rare but can be life-threatening.
Purpose of the Study:
- To report a rare case of Streptococcus mitis endocarditis.
- To describe the unusual complication of spontaneous splenic rupture secondary to splenic infarction.
- To emphasize the potential for severe intra-abdominal bleeding and mortality.
Main Methods:
- Case report presentation.
- Review of clinical course and diagnostic findings.
- Autopsy findings (if applicable, implied by death).
Main Results:
- Diagnosis of Streptococcus mitis endocarditis involving aortic and mitral valves.
- Identification of splenic infarction leading to spontaneous rupture.
- Massive intra-abdominal hemorrhage as the terminal event.
Conclusions:
- Streptococcus mitis endocarditis can present with rare and fatal complications.
- Splenic infarction and rupture represent a critical, albeit infrequent, sequela of endocarditis.
- Prompt recognition and management of endocarditis are crucial to prevent catastrophic outcomes.
