Summary

Substance use disorders frequently co-occur with borderline personality disorder (BPD). Lifetime prevalence rates for substance use disorders in BPD range up to 72%, with prescription substance abuse showing equal rates in men and women.

Background:

Substance use disorders (SUDs) are frequently comorbid with psychiatric conditions.
Borderline personality disorder (BPD) shows a high comorbidity with SUDs.

Purpose of the Study:

To review the empirical literature on the prevalence of SUDs in individuals with BPD.
To examine gender differences in SUDs among those with BPD, including prescription substance abuse.

Main Methods:

Literature review of eight studies.
Analysis of prevalence rates across different sample types (inpatient, outpatient, community).
Examination of current, 12-month, and lifetime prevalence rates.

Main Results:

Lifetime prevalence of SUDs in BPD ranges from 14% to 72%.
Lifetime SUD rates in BPD are higher than current or 12-month rates.
In clinical samples, men with BPD have higher rates of comorbid SUDs than women, except for prescription substance abuse where rates are equal.

Conclusions:

There is a significant comorbidity between BPD and SUDs.
Gender distribution in SUDs among BPD patients differs between general substance abuse and prescription substance abuse.
