Background:

Angel's trumpet (Datura suaveolens) contains natural alkaloids with anticholinergic properties.
Ocular exposure to plant saps can lead to various adverse effects.

Observation:

A 24-year-old man presented with acute, unilateral mydriasis (pupil dilation).
Detailed history revealed accidental ocular instillation of Angel's trumpet sap.

Findings:

The patient exhibited symptoms consistent with parasympatholytic intoxication.
Clinical presentation and history confirmed toxic etiology of anisocoria (unequal pupils).

Implications:

Sudden mydriasis can be a sign of toxic exposure, necessitating thorough patient history.
Angel's trumpet sap is a potential cause of acute mydriasis and systemic effects like tachycardia.
