  • Patients were randomized to either nonoperative management (immobilization, physiotherapy) or surgical MPFL reconstruction.

Main Results:

  • The surgical group showed significantly higher mean Kujala scores (88.9) compared to the nonoperative group (70.8; P = .001).
  • A greater percentage of 'good/excellent' results were observed in the surgical group (71.43%) versus the nonoperative group (25.0%; P = .003).
  • The nonoperative group experienced 35% recurrence/subluxation rates, while the surgical group had no reported instances.

Conclusions:

  • MPFL reconstruction using patellar tendon graft is superior to nonoperative treatment for primary patellar dislocation.
  • Surgical treatment results in better functional outcomes and significantly reduces the risk of recurrent instability.
  • Improved Kujala scores and absence of recurrence support operative management for acute patellar dislocations.
