Massive lower gastrointestinal bleeding in a young patient was successfully treated with extensive right hemicolectomy. This surgical approach controlled hemorrhage from arteriovenous malformation and early inflammatory bowel disease, offering a safe therapeutic option.

- Histopathological examination identified both an arteriovenous malformation and early inflammatory bowel disease lesions as causes of bleeding.
- The patient stabilized post-surgery with no further signs of hemorrhage.
- Extensive right hemicolectomy proved effective in controlling massive LGIB from dual pathologies.
Implications:
- Extensive right hemicolectomy is a viable and safe therapeutic option for massive LGIB of unknown origin in young patients.
- This approach offers lower mortality and morbidity compared to subtotal colectomy.
- Close postoperative monitoring is crucial for patients undergoing hemicolectomy for LGIB.
