Allogeneic hematopoietic stem cell transplant (HSCT) offers long-term disease control for advanced Cutaneous T-cell Lymphoma (CTCL). This treatment shows a graft-versus-lymphoma effect, with many patients achieving remission and remaining disease-free post-transplant.
Area of Science:
- Hematology
- Oncology
- Immunology
Background:
- Cutaneous T-cell lymphomas (CTCL) are incurable non-Hodgkin lymphomas.
- The efficacy of allogeneic hematopoietic stem cell transplant (HSCT) for CTCL is not well-established.
- Allogeneic HSCT may offer graft-versus-lymphoma (GVL) activity.
Purpose of the Study:
- To evaluate the outcomes of allogeneic HSCT in extensively pretreated CTCL patients.
- To assess the graft-versus-lymphoma (GVL) effect in CTCL patients undergoing HSCT.
- To determine the safety and efficacy of reduced intensity conditioning regimens in this context.
Main Methods:
- Retrospective analysis of 12 extensively pretreated CTCL patients.
- Allogeneic HSCT with a focus on reduced intensity conditioning regimens.
- Assessment of complete remission (CR), treatment-related mortality (TRM), and disease-free survival (DFS).
Main Results:
- Six patients achieved complete remission (CR) with active disease at transplantation, indicating a GVL effect.
- Treatment-related mortality (TRM) was low.
- 42% of patients were alive and disease-free at a median of 22 months post-transplant.
- Two patients demonstrated direct GVL effects upon immunosuppression withdrawal or donor lymphocyte infusion.
Conclusions:
- Allogeneic HSCT can achieve long-term disease control in advanced CTCL.
- HSCT provides a viable treatment option for CTCL refractory to conventional therapies.
- The GVL effect is a significant factor in the success of HSCT for CTCL.
