Idiopathic ventricular fibrillation (VF) is an underrecognized cause of sudden cardiac death in healthy individuals. Prompt diagnosis and antiarrhythmic therapy can improve outcomes and prevent reoccurrence.

Area of Science:

  • Cardiology
  • Electrophysiology
  • Sudden Cardiac Death

Background:

  • Idiopathic ventricular fibrillation (VF) is a rare condition.
  • It affects ostensibly healthy individuals without known cardiac or noncardiac causes.
  • This review consolidates data on idiopathic VF from existing literature.

Purpose of the Study:

  • To review and synthesize the characteristics of patients diagnosed with idiopathic VF.
  • To assess the diagnostic methods, electrophysiologic findings, and outcomes.
  • To evaluate the effectiveness of antiarrhythmic therapy and prognosis.

Main Methods:

  • Literature review of 19 articles reporting cases of idiopathic VF.
  • Analysis of 54 patients meeting idiopathic VF criteria.

Related Experiment Videos

  • Review of electrophysiologic study (EPS) data and treatment outcomes.
  • Main Results:

    • The mean age of affected patients was 36 years, with a 2.5:1 male-to-female ratio.
    • Over 90% of patients required resuscitation; syncope was common.
    • Electrophysiology studies showed a 69% inducibility rate for ventricular tachyarrhythmias, often polymorphic.
    • Class IA antiarrhythmic agents were effective in preventing reinduction.
    • An estimated 11% 1-year sudden death rate was observed.

    Conclusions:

    • Idiopathic VF is an underestimated cause of sudden cardiac death in seemingly healthy individuals.
    • Electrophysiologic studies can induce ventricular tachyarrhythmias in most patients.
    • Antiarrhythmic therapy appears to improve prognosis, highlighting the need for an international registry.
