Immunohistochemistry for detecting micrometastasis and isolated tumor cells in gastric cancer lymph nodes did not correlate with worse patient outcomes. These advanced techniques are not recommended for routine histological examination of lymph nodes.
Area of Science:
- Oncology
- Surgical Pathology
- Cancer Research
Background:
- Lymphadenectomy is crucial for gastric cancer treatment.
- Standard hematoxylin-eosin staining may miss small tumor foci (micrometastasis, isolated tumor cells) in lymph nodes.
- Immunohistochemistry offers enhanced detection of these occult metastases.
Purpose of the Study:
- To assess the clinical and prognostic relevance of lymph node micrometastasis and isolated tumor cells in gastric cancer patients.
- To determine if advanced staining techniques improve patient outcome prediction.
Main Methods:
- A prospective, multicenter trial involving 73 gastric cancer patients undergoing radical resection.
- Standard hematoxylin-eosin staining of all lymph nodes.
- Immunohistochemistry using cytokeratin antibody for all hematoxylin-eosin negative lymph nodes.
Main Results:
- A total of 1245 lymph nodes were analyzed, with an average of 17.3 per patient.
- Hematoxylin-eosin staining revealed metastasis in 29% of nodes.
- Immunohistochemistry identified micrometastasis in 3% and isolated tumor cells in 6% of lymph nodes.
Conclusions:
- The presence of lymph node micrometastasis and isolated tumor cells in gastric cancer patients was not associated with a worse oncological outcome.
- Immunohistochemistry is not currently recommended as a routine diagnostic tool for lymph node examination in gastric cancer due to its cost and complexity.
- Further research may explore specific patient subgroups where these findings could be relevant.
