Purpose of the Study:

  • To investigate the relationship between central NA and 5-HT function and the clinical characteristics of major depressive episodes.
  • To assess NA function via growth hormone response to clonidine (CLO) and 5-HT function via prolactin response to d-fenfluramine (d-FEN).
  • To classify depressed patients into distinct groups based on these neurochemical responses.

Main Methods:

  • Recruited 53 medication-free inpatients diagnosed with a major depressive episode.
  • Administered clonidine (CLO), an α2 NA agonist, to measure growth hormone response (ΔGH) as an index of central NA function.
  • Administered d-fenfluramine (d-FEN), a 5-HT releaser/uptake inhibitor, to measure prolactin response (APRL) as an index of central 5-HT function.

Main Results:

  • Four patient groups emerged based on CLO and d-FEN test responses.
  • A blunted prolactin response to d-FEN (Group 1) was associated with recent violent suicide attempts and mild anxiety.
  • A blunted growth hormone response to clonidine (Group 2) was linked to severe anxiety and no history of suicide attempts.
  • A combined blunted response (Group 3) correlated with a history of suicide attempts, longer illness duration, older age, and multiple hospitalizations.
  • A significant portion of patients (39%) showed no abnormalities in these neurochemical tests (Group 4).

Conclusions:

  • Specific psychopathological features in depression appear linked to central noradrenergic and/or serotonergic dysfunction.
  • These neurochemical dysfunctions are less likely primary causes of mood disorders.
  • Findings suggest these dysfunctions may reflect a failure in compensatory mechanisms crucial for affective homeostasis.
