Summary

Approximately one-fourth of patients with peritonsillar infections undergo tonsillectomy within five years. Young age and prior tonsil infections increase the likelihood of delayed tonsillectomy, even without initial surgical plans.

Background:

  • Peritonsillar abscess and cellulitis are common infections.
  • Tonsillectomy is a potential treatment, but its necessity after peritonsillar infections requires further study.

Purpose of the Study:

  • To determine the rate of tonsillectomy within five years following peritonsillar infection.
  • To identify predictive factors for patients who would benefit from tonsillectomy.

Main Methods:

  • Retrospective analysis of 809 patients (aged >6) with peritonsillar infections (ICD-10 J36).
  • Data collected on infection history, treatment, and subsequent tonsillectomy within five years.
  • Comparison between patients who underwent tonsillectomy and those who did not.

Main Results:

  • 159 patients underwent abscess or planned interval tonsillectomy.
  • 42.5% of conservatively treated patients aged 7-16.9 years required later surgery, compared to 31.3% (17-29.9 years) and 13.2% (>30 years) (p < 0.001).
  • Previous tonsillar infections increased the probability of delayed tonsillectomy (p = 0.067).

Conclusions:

  • About 25% of patients with peritonsillar infections ultimately undergo tonsillectomy within five years.
  • Younger age and a history of tonsillar infections are significant predictors for delayed tonsillectomy.
  • Factors like abscess re-opening, antibiotic use, or inpatient/outpatient status did not influence the need for later surgery.
