Sacroiliac joint vacuum phenomenon (SJVP) is common, especially in older females, but frequently underreported on CT scans. Radiologists often miss this finding, regardless of specialty or scan type.

Background:

  • The vacuum phenomenon (VP) in sacroiliac joints is frequently observed.
  • Its clinical significance in patients experiencing back pain remains a subject of debate.

Purpose of the Study:

  • To determine the prevalence of sacroiliac joint vacuum phenomenon (SJVP) on CT scans.
  • To assess the reporting rate of SJVP by body imagers and neuroradiologists.
  • To investigate if SJVP is reported more often on spine versus abdominopelvic CTs, and by neuroradiologists versus body imagers.

Main Methods:

  • Retrospective review of 652 patients' CT images of the pelvis and lumbar spine.
  • Analysis of axial thin-section images under lung and bone window settings.
  • Assessment of age, sex, and radiologist reports from electronic medical records.

Main Results:

  • The prevalence of SJVP was 34%, higher in females (41%) and older patients (39%).
  • Eighty-five percent of SJVP cases were bilateral.
  • Only 17% of identified SJVP cases were documented in radiologist reports.
  • No significant difference in reporting rates was found between body imagers and neuroradiologists.

Conclusions:

  • Sacroiliac joint vacuum phenomenon is a prevalent finding, particularly in older and female populations.
  • SJVP is significantly underreported across both abdominopelvic and spine CT studies.
  • Current reporting practices by radiologists do not differ significantly based on specialty or scan type.
