Influenza A (H1N1) pneumonia cases in Italy showed a milder course and lower mortality compared to global trends. However, a significant portion of patients developed severe respiratory failure requiring mechanical ventilation.

Conclusions:
- Influenza A (H1N1)-associated pneumonia in Italy presented with a milder clinical course and lower mortality than reported internationally.
- Despite milder overall trends, a substantial percentage (33%) of cases rapidly progressed to respiratory failure.
- Mechanical ventilation was a necessary intervention for a significant subset of patients.
