Background:

  • Assessing bone density changes around dental implants is crucial for predicting long-term success.
  • Digital subtraction radiography (DSR) offers a quantitative method for evaluating these changes over time.

Purpose of the Study:

  • To quantitatively assess longitudinal bone density changes around single-tooth implants.
  • To compare bone density variations between immediate loading (IML) and conventional loading protocols.
  • To evaluate the influence of implant surface treatment on bone density.

Main Methods:

  • 12 patients received standard and TiUnite® implants with either immediate or conventional loading.
  • Standardized periapical radiographs were taken at baseline, 3, 6, and 12 months.
  • Digital subtraction radiography (DSR) and Image Tool software were used for quantitative bone density analysis.

Main Results:

  • Bone density, measured by grey levels, was significantly affected by follow-up time and loading protocol.
  • Immediate loading (IML) implants showed a linear increase in bone density.
  • Conventional loading implants exhibited a significant decrease in bone density at 12 months.

Conclusions:

  • The immediate loading (IML) protocol stimulates mineral bone gain around single-tooth implants within the first year.
  • Implant surface treatment did not demonstrate a significant effect on bone density changes.
  • IML may be a favorable option for bone preservation in suitable clinical cases.
