Background:

  • World Health Organization (WHO) body mass index (BMI) cut-off values may not accurately represent obesity across diverse populations.
  • Direct measurement of percentage body fat (PBF) offers a more precise method for diagnosing preobesity and obesity.

Purpose of the Study:

  • To analyze body composition in an adult population in Centre-South Italy, stratified by age and gender.
  • To assess the agreement between BMI and PBF cut-off points for health status classification.

Main Methods:

  • Utilized Dual X-ray Absorptiometry (DXA) for anthropometry and body composition assessment.
  • Included 3,258 adult participants from a total pool of 4,408.

Main Results:

  • Prevalence of "at risk" subjects (preobese and obese) was 69% for men and 85% for women based on PBF.
  • Low agreement was found between BMI and PBF categories for the total, male, and female populations.
  • BMI showed a stronger false negative classification for women and younger individuals.

Conclusions:

  • Screening for adiposity using PBF can identify individuals with normal BMI who are at higher risk for cardiometabolic issues.
  • Established PBF cut-off points, by age and gender, can serve as a valuable reference for clinical and public health settings, particularly for the Italian Caucasian population.
