Fatty meal sonography

Purpose of the Study:

  • To prospectively evaluate the effects of cholecystectomy on biliary dynamics using fatty meal sonography.
  • To assess the reliability of fatty meal sonography in identifying the need for common bile duct exploration.

Main Methods:

  • Common hepatic duct diameter was measured via ultrasonography before and after a fat stimulus in 25 patients pre-cholecystectomy.
  • Measurements were repeated 1 month, 1 year, and 5 years post-surgery in 23 patients.
  • Preoperative and postoperative responses to fatty meals were analyzed.

Main Results:

  • Preoperatively, fatty meal response was a poor indicator for common bile duct exploration.
  • Postoperatively, the response was variable at 1 month and 1 year, with over half showing no duct size decrease.
  • A more consistent decrease in common hepatic duct size after fatty meals emerged at 5 years.

Conclusions:

  • Fatty meal sonography is unreliable for predicting common bile duct exploration needs preoperatively.
  • Post-cholecystectomy, the response to fatty meals is unpredictable, especially in the early postoperative period.
  • Measuring the common hepatic duct is preferred over the common bile duct, with 1 mm changes likely insignificant.
