Area of Science:

  • Pharmacokinetics
  • Analytical Chemistry
  • Tuberculosis Research

Background:

  • Isoxyl, a potential treatment for multi-drug resistant tuberculosis (MDR-TB), was discontinued due to clinical failures.
  • Understanding isoxyl's in vivo behavior is crucial for its potential repurposing.

Purpose of the Study:

  • To develop and validate a reverse-phase high-performance liquid chromatographic (HPLC) method for quantifying isoxyl in plasma.
  • To investigate the in vivo plasma concentrations of isoxyl in guinea pigs.

Main Methods:

  • Isoxyl extraction from guinea pig plasma using acetonitrile.
  • Quantification via HPLC with UV detection (270 nm) using a Spherisorb ODS2 column.
  • Method validation according to FDA guidelines, determining recovery (>68%) and limit of quantification (0.25 μg/ml).

Main Results:

  • A sensitive, reproducible, and accurate HPLC method was established.
  • Isoxyl plasma concentrations in guinea pigs were significantly lower than the minimal inhibitory concentration (MIC) required to inhibit Mycobacterium tuberculosis.
  • The method demonstrated no interference from plasma impurities or internal standards.

Conclusions:

  • The developed HPLC method is suitable for bioanalytical determination of isoxyl in plasma.
  • Extremely low in vivo isoxyl plasma concentrations likely contributed to its clinical failure in treating MDR-TB.
  • This research provides a foundation for further pharmacokinetic studies of isoxyl.
